Systematic Sloppiness

Anders Sandberg writes in Practical Ethics.

In his article It is 10 o'clock, do you know what your cells are? he describes the amazingly persistent problem of contaminated cell lines. It has been known for more than 40 years. This is a nasty example of how systematic sloppiness can become in human institutions and how it might threaten research.
